New Patient Registration

registrationTo register with the practice you must live within the practice area.  It is practice policy that you are registered with a named doctor.  However, you may express a preference to see another doctor when you book an appointment. Whilst the practice will try and comply with your request it may not always be possible for you to see the doctor of your choice.

The practice does not discriminate on the grounds of race; gender; social class; age; religion; sexual orientation or appearance; disability or mental condition.

All new patients must provide identification before they can be registered eg one item of photo ID and a bill with your current address. In welcoming you to our practice, each member of the family will be asked to complete a health questionnaire and new patient information sheet. Please ensure that you give full details, as it may take some time for your medical records to arrive from your previous doctor.

You will be asked to complete a new patient questionnaire documenting your medical history. This will ensure that information is available for reference initially, until we receive records from the previous doctor. If you are on repeat medication you will be asked to make an appointment with a doctor prior to your current prescription running out.

For children under five it is useful for details of their vaccination history to be recorded from the red book that you hold for them.
