Patients might want to consider putting together a future care plan. There is a basic covid-19 plan attached that you could review and update or you can make more detailed decisions using the My Decisions and My Directives website:
If you do create or update your plan, please share it with us, preferably by emailing it to the surgery on E82075.regalchamberssurgery@nhs.net or by posting a printed copy to the surgery, so we can record the plan in your notes. This will mean your preferences will be available to us, and the district nursing services.
Hospital services may not be able to access our record, so please ensure that you have a paper copy, or a copy on your mobile device to share with hospital services of needed.
